Self Pay Rates
If you would like to receive treatment from CTS Physical Therapy, but your insurance company does not cover the cost, you may use an additional option of self pay.
When would you use self pay?
-If you or your family have no insurance
-If you have a large deductible which you do not anticipate meeting by the end of the calendar year
-If your insurance plan only covers a minimal number of physical therapy visits per year but you need or want more
-When you don’t want your medical information to be reported to the insurance company
Option 1 – Consultation & Treatment
$100 per hour
$25.00 per 15 minute additional increment
$100.00 – 60 minutes$125.00 – 75 minutes$150.00 – 90 minutes
- We can do your first visit – which will be a consultation & treatment – without a MD order. Due to TN state law, we have to have a MD order for any further visits.
- Additional increments can be added at the scheduled appointment if the therapist schedule allows for the extra treatment time.
- There cannot be a lapse of more than 30 days between visits or your MD order will become void.
- No insurance filed.
Option 2 – Consultation (2nd Opinion)
$50.00 for 30 minutes
- No Doctor order required
- No insurance filed
- No treatment rendered
- If new patient, you will also watch a 20 minute video with Piotr Ulmer, PT explaining CTS’s manual therapy
- Maximum 1 x per 8 weeks
Option 3 – Prebuy
Pre-buy 4 visits at $90 per hour$22.50 per 15 minute additional increment
$90.00 – 60 minutes$112.50 – 75 minutes$135.00 – 90 minutes
- The pre-buy rate only applies if indicated PRIOR TO the scheduling of the appointment.
- Additional increments can be added at the scheduled appointment if the therapist schedule allows for the extra treatment time. The increments do not have to be scheduled/purchased prior to treatment as the hourly treatment does.
- There cannot be a lapse of more than 30 days between visits or your MD order will become void.
- The is NO REFUND for visits which are purchased and are not used, but can be transferred to another person to be used within 60 DAYS. (____ Initials)
